Job Description: Should be able to study patients Medical condition and design care plans Conduct care team meetings Internal monitoring of the data and suggesting tactics to improve care and achieve practice goals. Should be able to analyze patient data recorded in software and define the care plans for high-risk chronic disease patients. Define Risk level of patient population based on their Medical condition. Should be able to Communicate with Doctors Office on phone call or via email to coordinate the patient care. Attend the virtual learning session and enhance the project knowledge and disseminate it within the Team Requirement / Qualification: Education Background - Graduate Nurses, Medical Officer or Other Medical Background Proficient in using Computer and Software Tools Students Preparing for USMLE are preferable but not mandatory Good Verbal and written communication skills Quick Learner Shift Timings: Day Shift- 8:30 AM to 5:30 PM Day-Noon- 12:00 PM to 9:00 PM Day-Noon- 1:00 PM to 10:00 PM Day-Noon- 2:00 PM to 11:00 PM,
