Roles and Responsibilities Job duties of this position include but not limited to the following: 1. Complete Intake Assessments of leads/ prospects. 2. Client Need analysis 3. Design care package and finalize the Care contract. 5. Coordinate Start of Care with Operations team. 6. Coordinate to arrange Durable medical equipment as needed. 7. Enter Leads data in SHC on daily basis as leads are processed by RM 8. Maintain & document minimum once a week relationship contacts with active patients as assigned. 9. Meet and follow up with referral sources as assigned by SRM. 10. Staff Company Kiosks at assigned hospital as needed. 11. Participate in Health Fare events as assigned & scheduled.
